fix: consolidate hooks into hooks/, update AGENTS.md
Move pre-commit hook from .githooks/ to hooks/ alongside existing
prepare-commit-msg. Fix install-hooks script to use `hooks` path.
AGENTS.md: document pre-commit hook (README version sync), fix stale
"16 steps" → 20 steps in Install Orchestrator section.
