Operator & Codex 16bfacb4ef Add database live grounding probe (Sam & Codex) 
Wire the database explanation domain to a best-effort psql database-list probe using the configured ops database connection. The probe passes PGPASSWORD through exec env instead of argv, stays on the binary allowlist, and degrades to an unavailable marker on auth, timeout, or command failure.
